My learning through posting

In my clinical  posting in male ward i came across a patient ,42 yr male from Dahalwada dist. Narsinghpur  having chief complaint of decreased hearing problem since 1 month from his date of admission and pain in chest and lumbosaccral region.
         
On further investigation about his problem of decreased hearing i suspect that it may be due to following causes-:
1)Obstruction in his auditory tube.
2)Stiffness of tympanic membrane.
3)Obstruction in conduction of sound waves in ear ossicles.
4)Nerve damage,hair cells damage.
On examining patient's ear through autoscope hole in his both tympanic membrane was clearly visible.

On searching of the possible treatments of TM perforation i came to the result that it could be corrected through myringoplasty,tympanoplasty,one more interesting thing that i found ws tympanoplasty could be done by using fibrin glue&abstracting human fibrin instead of using fascia but success rate is less by this method.

Investigating about his second problem i.e.chest pain,the anatomical basis was normal(Xray) and we found that possible physiological cause of this  was enthesitis.

Overall i got much to learn by this case.
